Consumer had the procedure approximately 18 months. Frieda was told by the consumer he was arrested in [redacted] for a [redacted] and was terminated from his job and only requested a refund to pay for the court associated fines.

He had a total of at least two procedures and was seen at least four times for a consult.
The business will supply documentation if the consumer signs a HIPAA release form.
