I am rejecting this response because:  I have no documentation that Kozik's contacted Medicare.  In fact, the opposite is true.  Medicare indicated Kozik's did not contact the provider line or Medicare to supply information needed by Medicare to resolve and reimburse the customer.  I stand by my initial complaint in full.  I intend to pursue the matter and Kozik's fraudulent business practices with Medicare and the media.  I reject the response.
