This letter is a response to complaint #[redacted]. On July 11,2015, Saturday Mrs. [redacted], [redacted] hadan appointment with Dr. ** to be seen for daily routine cleaning. Mrs. [redacted] was notified that theprevious insurance on file was no longer active and that new insurance was needed. Because...
herinsurance had changed we asked her to provide us her new insurance policy information via multiple textson Friday, July lOth at I 1:52am. Mrs. [redacted] did not respond until Friday, July lOth at 6:20pm, after herinsurance company closed for service. She also provided us with incomplete and incorrect information.Due to this incomplete information, we were not able to verify insurance information.Our office does take [redacted] insurance, so we told her we took her insurance. We did not, however,confirm any personal coverage for her because of her failure to respond to our multiple requests for herinsurance information.On Monday morning when the insurance office reopened, our office inquired about the specifics of thepatient's dental insurance coverage. Unfortunately, our office was unable to accept her specific insurancepolicy because she had switched from her previous PPO to a new HMO, which we do not take.The client then disputed the charges with us and filed a grievance with your office that same day withoutattempting to settle the matter with our office.Dr. ** had waived her fee even though he legally has a right to pursue his fee with her. We then receivedher complaint with Revdex.com on July 17th, though he had waived her fee for the day of service.Thank you,
