Carondelet incorrectly billed in-network service as out of network in early This was corrected, but now they are attempting to re-bill I saw my personal care physician in Jan, for a routine checkup Carondelet later billed me over $which I reviewed with my insurance company (United Health) and discovered that the service was coded as out of network A visit to your in-network PCP is anything but out of network I called Carondelet in about March of last year and I believed that this error was corrected and was told that I owned nothing, having paid the co-pay at the office on the date of service Now I have a bill from Carondelet for $for the service provided in January after having been told that I owed nothing

I am unable to get a copy of my mammogram report (from 2013) and images sent to SimonMed located at *** * *** ** *** *** *** ** XXXXX They have sent three requests since December of Most recent being 02/10/ I am facing breast cancer and Carondelet is ignoring the request for medical records They also refuse to mail these to my home address in Phoenix I can not take time off from work to drive over miles to get my records
