Refused to provided copy of patient records to me in order to seeking care at other facility even I agree to sign their release form. When I came to this office I have transferred a complete X-rays for use without further X-Ray is needed for my general cleaning. The office claimed that they took some additional X-Rays without asking for my consent and failed to informed me of any additional charges will incur. Additionally, this dental office doesn't clean on the outside of my teeth and charged for it. The office always informed me of my portion of the bill and my insuracne company portion of the bill and I always pay my portion of the bill before leaving the establishment. The office has never informed me about additional X-rays nor informed me of additional fees relating to the X-Rays that I don't need nor required. The accusation of owing additional fees only occur after I have discontinue their service due to substandard care and service. I have asked in writing to send me a copy of the authorize release form so that I can sign to request a copy of my records to be sent to other care facility--the office refused to send me the form. They claimed that I owe them fees for X-rays, the X-Rays that I have never been informed of nor required, and never inform me of how much the fees was until I have decide to discontinue their lousy service and substandard of care.
