On 6/7/a request to release records was received by our office from [redacted] and [redacted] *** The request was to release records to [redacted] On 6/7/the most recent radiographs were electronically sent to the above mentioned email address On 6/27/ [redacted] came to our office demanding her records I explained an email had been sent on 6/7/16, the day the request was received to [redacted] as requested I called [redacted] , Dr [redacted] 's office to verify receipt of those images I spoke with [redacted] on 6/27/asked if she could verify the images were received for both [redacted] and [redacted] ***, [redacted] immediately replied "yes!" I then proceeded to make photocopies of all written records, print digital radiographs and photos that were taken while [redacted] and [redacted] were patient's of record with Dr [redacted] I had a military record for [redacted] that I released back to her [redacted] thought [redacted] also brought a copy of his military record to us when he came for his initial visit I promised I would continue to look in storage where our inactive records are stored I have been unable to locate that particular record I also explained to [redacted] that we did have "transferred" images from 1/28/which was prior to ***'s initial visit with Dr [redacted] I asked if it was possible his military record was not brought to our office, rather a transferred copy emailed to our office instead?
