the consumers medical information due to privacy law, though I am able to disclose that the consumers notes entered in by customer service and communication with the consumer reflects a highly responsive effort to secure *** *** orderThe problem revolves around the physicians completion of a form classified as a *** which is-required-to-establish medical necessity An Rx or prescription as we are most familiar with works in conjunction with the *** though; it is the; *** that is required by the State of Pa., which is an absolute requirement.Interphase staff communicated this issue-revolving around- the physician incompletion of the *** .The physician failed to sign it and we have made several attempts to contact the physician and have him sign where indicatedOn 9-22-16, staff contacted *** *** again to have him: please contact his physician and 'have him appropriately sign the ***.Interphase Medical continues to work diligently with *** *** in assisting: him to obtain the necessary enteral products Once we have the appropriate documentation from *** ***'s physician we can forward to the State for medical determination Please contact me if you require additional information (ID ***)Thank you,Glenn MVice President
