months, therefore:Your scheduled payment of $57.25 that was due on September
27, 2015 would serve to satisfy your 30 day cancellation term as stated in your
membership agreement.An early termination fee of 20% of the total value of your
agreement (total agreement value: 12 x $49 = $588) is due upon cancellation.  This amount is $117.60.Finally, you chose to finance your $99 enrollment fee over
12 months.  The remaining 5 payment of $8.25
would be due upon cancellation.  This
amount is $41.25.The total amount due to terminate your agreement is $216.10.You do still retain the option of freezing your membership for
a period of time determined by a medical physician.
