I am responding to the letter that we have received from your office regarding [redacted] , I have enclosed for your review all pertinent information regarding her caseWe have tried to work with the patient and we have refunded her at total of $on April 20, [redacted] had extensive dental work done at our office including extractions, Implants and a bridgeThe patient’s insurance did not cover any of her expenses, I have enclosed her treatment plan, EOBs and transactionsEven though we did not receive insurance payments, we processed write offs on her behalf leaving a credit balance on the initial treatmentThe patient then started treatement on a bridge, I have enclosed the plan she signed and it does state that the Total is $4801.00, patient pay of $ [redacted] had a credit from previous work, which we applied, and her amount out of pocket was $which she paidWe did receive a payment from the insurance company of $There is no money due for the bridgeAt this point we are in the final stages, wax try in, then placement but the patient is refusing to finish treatment and we still have no idea whyShe has stated that she is fine with the Doctor working on the case, but she will not come backWe have paid the lab bill and have expenses tied up in this case.SincerelySandie F
