Dear [redacted] ,This letter serves as a response to complaint ID [redacted] dated August 3, After careful review of the complaint the findings are as follows:The bill for date of service 10-21-in the amount of $was billed correctly with the right insurance, The bill was paid 11-12- and a statement went out to the patient 11-13-15.The bill for date of service 10-29-was billed incorrectly by us.As far as applying for Financial Assistance the patient did not supply all the information neededThe Financial Assistance Counselor sent an email recently to the patient asking for the information that was not previously sentIf all the paper work is returned they will check to see if she is eligible for any type of assistance.We have pulled both claims from our pre collections company.Regards,Jan W.Revenue Cycle Supervisor

and a statement went out to the patient 11-13-15.The bill for date of service 10-29-15 was billed incorrectly by us.As far as applying for Financial Assistance the patient did not supply all the information needed. The Financial Assistance Counselor sent an email recently to the patient asking for the information that was not previously sent.
