highest quality of health care experience possible, and take pride in committing to each of our patient’s oral health. Upon reviewing the case, it appears the patient presented himself at the office on a $59 value for an exam, xray and cleaning. At that time, the patient was diagnosed with periodontal disease which involves below the gum line cleaning in order to treat the infection. The patient returned in January 2015 for periodontal maintenance cleaning. It seems there may have been a misunderstanding of treatment along his treatment journey, as the office was not aware of the patient concerns. The Dr and office welcomes the patient into the office to discuss his concerns. In regards to the adjustment, as an office, we are contracted with the patients insurance. The treatment plan that we provided is an estimate, and we don’t truly know the benefits of the insurance until we get the EOB. We made adjustments to the lower cost, so that the office is in compliance with the insurance contract. As requested by the patient, below is written breakdown of services rendered: DOS 1/2/15 – Patients portion was $155.12 - $59 for [redacted], $29.12 [redacted], and $67 for [redacted]. There was a $50 deductible applied for the 2015 dental benefits year.  Patient paid $102.64 which left a balance of $52.48.  We apologize for any confusion this has caused the patient. The patient requested a $200 billing adjustment, we would love to discuss this further, and the door is always open to discuss these concerns in regards to the adjustment. Again Dr. and the office welcome the patient back in to discuss his treatment.
