In regards to Ms. [redacted] ***'s refund, a check for $30 was issued and mailed to her home address on 02/25/2015. With so many insurance companies and insurance plans, it is not the responsibility of providers to know what the patient's benefits are when it comes to health care, specially... preventive services. Some insurance companies require that a copay be paid and others do not. At the time of visit, Ms. ***'s card clearly stated a $30 copay was due for an office visit. Going back into the records, there was only one phone call requesting the refund. We apologize that it took this long for the refund to be issued.
