I have been a patient at St Vincent Morrilton, Morrilton, Arkansas XXXXX.My payments are mailed to St Vincent Health Systems, ** Box [redacted], Memphis, TN XXXXX.I telephoned St Vincent Health Systems requesting itemized statements for my two accounts which includes dates of payments credited to the accounts, amount of each payment received, and the remaining balance after each payment is applied. I also requested a monthly statement because I have never received a monthly statement.Also, five payments totaling $200.00 have not been paid by my bank. I inquired if these payments have been received. I have been advised by my bank if these payments have not been received, an immediate investigation will be done.I was told to put my request in writing. A letter dated November 9, 2015 was mailed to St Vincent Health Systems.As of the filing of this complaint, I have not received a response.Desired SettlementI would like an itemized statement of all payments received, a monthly statement for both my accounts and information concerning payments that have paid but have not been paid by my bank
