To Whom It May Concern,We did receive a check from the patient’s insurance companyHSA received the check on November 03, 2015. Some of the charges were paid by the insurance carrier; however the office visit charge, for which the copay was paid, was denied. After review, we believed
that charge should have been paid by the insurance companyWe therefore appealed the denial of the charges to the insurance company for payment of the office visit charges. We received notice that the denial was upheld on 01/27/2016. A credit for the copay was then created on 01/27/2016. It was forwarded to the collections department on 02/01/2016. A refund request was received by the manager on 02/04/2016. The refund was approved on 02/07/and a check was issued and mailed the same daySincerely,*** ** ***Director of Business IntegrationsHand Surgery Associates
