To whom it may concern: This is in response to case ID [redacted] regarding [redacted] I have pulled a copy of Mr [redacted] ' signed contract (attaching a copy) He signed the contract on 11/23/2016, it was for a $membership He paid $on that day His membership dues were due on 12/6/2016, his credit card was declinedABC our billing company tried to charge his credit card again on 12/9, 12, 14, 16/On 12/16/the card with through, and they added a $service fee The total amount deducted as $($dues plus $service fee) We do not take cancellations over the phone, so when Mr [redacted] called to cancel he was told he would have to come in I am sorry that Mr [redacted] had a problem with management when he came in to cancel That should have never happened I have instructed the Operations Manager to refund Mr [redacted] $and to cancel his membership, the refund and cancellation were processed on January 5, I have also instructed Management to contact Mr [redacted] with an apology, a message was left on Mr [redacted] answering service If you have any questions please give me a call Our members are very important to us, I do everything I can to take care of them PJ Jones Get Fit VP 859-582-

We do not take cancellations over the phone, so when Mr. [redacted] called to cancel he was told he would have to come in. I am sorry that Mr. [redacted] had a problem with management when he came in to cancel. That should have never happened.
