Fitness SF mailed a price increase post card to Mr. [redacted] in May 2015 advising the rates were going up. This was mailed to his last known address on file:

on 7/31/15, as a courtesy, Fitness SF will not charge him for August dues. Fitness SF does not refund any prepaid dues as it states on his signed agreement. I have attached his membership agreement, front and back and price increase card mailed to him in May.
