Attached is the membership agreement of the above mentioned complaint filed by the customer.

[redacted] has had a joint membership with her daughter [redacted] since 03/2015 at Fitness Nation. On 09/03/2015 [redacted] terminated her membership and signed-up a separate membership for her daughter [redacted] She paid $10.00 Plus Tax $10.80 for the last month dues for [redacted]...

[redacted]. We charge our monthly dues on the 1st of every month and have an annual Gym Fee in October every year which is $20.00 + Tax $21.60. She was charged $21.60 on 10/1/2015, however it takes a few day for us to know whether the payment was rejected by the bank. In the meantime [redacted] came in at our Arlington location and cancelled [redacted] membership. The payment was rejected by Jacquelin's bank on 10/05/2015. Hence there was a return transaction fee assessed on the account for $10.00, which made the total outstanding balance $21.60 + 10.00= 31.60. This charge was outstanding from October while the membership was terminated. On 11/13/2015 she was charged $31.60 which was the outstanding balance. The membership was terminated in October and this is the final charge which was returned by her bank in October.
