of the contest itself.  The “Weight Loss Contest” was a 8 week contest were everyone that signed up was put on a team lead by 1 or more Instructors while being monitored for those 8 weeks through weekly weigh-ins and team meetings/workouts, all those that participated also received 2 half hour private training sessions.    To recap [redacted] signed a month to month agreement for $89+tax a month paying $299+tax upfront to cover her for the length of the contest plus all the extras the contest included, it then continued as a month to month for $89+tax a month that she approved to in the signed agreement which she could have cancelled at any time for no charge which she didn’t until May [redacted] 2016 when signing and handing in a Cancellation Form (which is attached)   Gregg B[redacted] General Manager UFC Gym Commack (LA Boxing)
