Dear [redacted] ,In response to the letter/complaint received, the following facts are documented in our systems and notesThe patient presented 11/7/for a medical examination and the insurance information was confirmed at that visit as up to dateThe bill was submitted to the insurance company and we received an explanation of benefits 12/26/stating “INSURANCE NOT IN EFFECT” indicating that the insurance was not validThe patient’s mother was called on 1/5/at the phone number on recordShe did not answer and her mailbox was fullAdditional phone calls were placed 1/24/and 2/16/to obtain updated medical insurance information, messages were left to update the medical insuranceThe patient’s mother called the office 2/24/and indicated that she did not have valid medical insurance for her sonThe total bill was $and she was offered a discount to $(37.5%) if she paid the bill immediatelyShe declined to do so and she was informed that she would be responsible for the full amountSincerely,Alex [redacted] M [redacted] , MD, PhD

company and we received an explanation of benefits 12/26/2016 stating “INSURANCE NOT IN EFFECT” indicating that the insurance was not valid. The patient’s mother was called on 1/5/2017 at the phone number on record. She did not answer and her mailbox was full. Additional phone calls were placed 1/24/2016 and 2/16/17 to obtain updated medical insurance information, messages were left to update the medical insurance. The patient’s mother called the office 2/24/2017 and indicated that she did not have valid medical insurance for her son. The total bill was $240 and she was offered a discount to $150 (37.5%) if she paid the bill immediately. She declined to do so and she was informed that she would be responsible for the full amount.
