I am writing as a former patient of DrHayden P [redacted] while she was affiliated with DrB [redacted] practice Executive Health Group, located at [redacted] *** I have requested via several phone calls and by letter (Certified Mail) to DrB [redacted] to receive a pro-rated refund of the $ [redacted] annual retainer I paid for the time of DrP [redacted] service that began September 11, and ended March 14, I was a former patient of DrP [redacted] while at [redacted] *** When I signed up with Executive Health Group I specified DrHayden P [redacted] on the EHG form as my Primary Physician According to the AMA Journal of Ethics dated July 2013, Volume Number 7:581-586, Opinion 8.005-Retainer Practices, under section (1) last sentence: “Patients must be able to opt out of a retainer contract without undue inconveniences or financial penalties.”
