Date of routine eye exam 12/10/12. Received billing within 3 weeks stating I owed full amount and insurance covered basically $12.

I had to spend countless hours communicating with their terrible billing dept over a 4 month time span and even after conformation from my insurance carrier that they were paid (once [redacted] coded it properly), I still to this day get phone calls from their office harrrassing me to pay them for the routine eye exam which should be covered by my insurance.

I would stay away from this practice; they are terrible!
