RE: Transfer of Medical Records
Dear Sir/Madam:
I am contacting you for your assistance in a matter where my former medical physician is withholding my medical records, and delaying me from receiving the care I need from my current physician.
Dr. Edith Grannum M.D of 1815 South Clinton Ave, Suite 330, Rochester, NY 14618 was my primary care physician from January 1, 2011 to December 31, 201 1. On December 12, 2011 I visited Dr. Grannum’s office for a regular visit. I paid $15.00 as a co-payment, per my agreement with my health care insurer United Healthcare.
On February 2!, 2012 I received a voicernail from Dr. Grannum’s office staff member *, stating that I am to pay $5.00 as a balance of my co-payment for my visit on December 12, 201 L She also stated that Dr. Grannum‘s would not release my medical records to my new primary care physician‘s office, unless I pay the $5 balance.
On February 22, 20l2 I returned the call to Dr. Grannum’s and spoke to *. I requested her to send me an official billing statement in order to make the payment of $5.00. I also requested the to send me a statement in writing stating why they cannot send my medical records to my current physician. On the same day I contacted United Healthcare by phone to advise them of this matter and inquired about the co—payment balance. United Healthcare assured me this was not an error on my part, they confirmed my co-payment of $15.00 was correct for that visit, and than it would take 60 days to correct this error.
On February 23, 2012 I called Dr. Grannum’s and spoke to * again about this matter, and she failed to respond to my request for billing documentation and letter stating refusal to release medical records.
I am now asking for your help in looking into this matter and solving this dispute. I need an official billing statement from Dr. Grannum’s office, so that I can make the payment of$5.00, and have a record of this transaction for any eligible claims and a copy for filing my taxes.
I feel that the actions of Dr. Grannum and her office staff have been extremely unprofessional and unethical in this matter and on other occasions. During every visit Dr. Grannum asks whether I have money. This leads me to believe the doctor is more concerned about making money, than providing care to her patients. As of today my current physician’s still has not received my medical records. These are my personal medical records; they do not belong to Dr. Grannum. I am a diabetic and partially handicapped. Dr. Grannunfs refusal to release my medical records is preventing me receiving the health care services I need.
