I am rejecting this response because: you have NEVER called Jay back or sent an itemized bill for his services. He personally went into the location several times asking for an itemized bill and was told they could not provide one but would get ahold of him from corporate - Never happened! I only got involved when he could get noplace. We will not dismiss this claim until Jay is holding an itemized bill with codes for the treatment he received not a bill for $660 for the proposed treatment plan.
