Mr. [redacted] had a schedule appointment on March 3, 2015. Prior to his appointment we tried to contact Mr. [redacted]; we needed to reschedule his appointment. Dr. Cheng recommended Mr. [redacted] have extractions and full upper and lower dentures. At the time of the recommendation Mr. [redacted] was...

A pre-authorization was support and approved.However, at the time of Mr. [redacted]'s appointment on March 3,2015 his insurance changed and he was then covered by [redacted]. The pre-auth from [redacted] Plan was no longer valid. A new pre-authorization had to be submitted to [redacted]. Again, we tried to reach Mr. [redacted] prior to his appointment to avoid an unnecessary trip for him. Upon his arrival for his appointment (on March 3rd) we tried to explain to him that his insurance changed and we had to send in a new pre-auth to his current insurance provider. Mr. [redacted] did not understand and got frustrated and left our office.We did send in the pre-authorization for complete his treatment if Mr. [redacted] wishes to complete it here.If you have any questions, please contact me at ###-###-####. Review: They refused to render services after already having a complaint filed against them with my insurance company. As a result, after the issue was supposed to already have been settled, they still refused to render services, even after my insurance case worker spoke to them and gave the pre-authorizationDesired Settlement: I wanted my dentures.
