Dear Mr***,Please call our offices at your earliest convenience so that we may extend your COBRA election period to coincide with the date you were able to pick up your Certified Mail which was delivered by the post office (according to their records) on September 1st. We mailed
this same notice to the address your employer had on file (* *** *** *** ** ***) on June 20, but it was returned to us by the post office with the message "Not Deliverable As Addressed Unable to Forward". We then verified that the address we were using was the address known at the time by your employer so we mailed again on July 8th with the same result. On August 23rd we mailed it Certified Mail but it was returned to us on August 27th from the post office with the message that they were "Unable to Forward". At that time, a new address was received of *** *** *** *** ** *** and this notice was sent Certified on September 1st and picked up on September 15th. We can certainly understand the confusion and want to give you the opportunity to make your election so please contact our offices at *** and ask for the COBRA Department. Again, we apologize for the delay
