I am rejecting this response because: I made appointments for skin exams with Crutchfield Dermatology and advised that I have a history of basal cell carcinoma By scheduling the appointments, Crutchfield Dermatology agreed to provide services expected of a skin exam DrCrutchfield misdiagnosed two spots that were basal cell The skin exam did not include a scalp exam; a basal cell spot was missed on the scalp because of the inadequate exam The assistant did not mark a spot because she couldn't see it; it is basal cell I pointed out a spot on my lower eyelid; DrCrutchfield looked at it from across the room and said it was eczema; it was a cyst I am being treated for eight basal cell skin cancers, including the spots misdiagnosed or missed by DrCrutchfield because his skin exams do not follow the guidelines of the American Academy of Dermatology At the conclusion of the appointment, DrCrutchfield asked if there was anything else as he was walking out the door - a clear indicator that the appointment was over I didn't say I was satisfied I just responded 'no', because it was clear at that point that the appointment was over
An e-mail to us indicated that we need to respond in days, or be suspended How do I respond? We have written off the $charge, removed it from the collection agency and reached out to the customer to let her know this has been done Please let me know what else needs to be done Thanks, Tim ***copied and pasted from email
I am rejecting this response because: This is the second time that I am attempting to correct thisMy 10-year old daughter now has a "bill" that has gone to collections due to a 'No Show Fee'We never confirmed the appointment, now did we sign any documents agreeing to this chargeAs I mentioned previously, Last summer when I made the appointment, I was told that if I did not call back with my credit card information, then the appointment would be cancelledI did not feel okay with this policy, and therefore never called back to provide my credit card information The desired outcome is that this charge will be taken away and that my daughter's information will be removed from their system
We reviewed Ms***'s concern and waived the "No Show" Fee on November 4, 2016. Please let me know if you need any additional informationThank you
When I called them, I left a message for someone (I no longer remember who); and never received a call back. Now, my daughter has received a letter from collectionsI called the collection agency and spoke to Rob; explained to him that the person he is trying to file collections on his years old! He suggested that I call Crutchfield and speak to Theresa in the billing department. I called her, as suggested, and left her a voicemail explaining the situation - including my daughter's information and reminding her that we never signed any agreement for billing procedures or no-show charges. I also let her know that I had filed a complaint with the Revdex.com. To date, I have not received a return phone call/voice mail from her

clinic alleging dissatisfaction about treatment received by a subsequent physician our clinic referred the patient to for Mohs surgery. That complaint does not appear in the one the patient filed with the Revdex.com.Our records show that during the patient's treatment, the patient was properly examined and diagnosed. After carefully examining the patient with special attention to the locations the patient identified, Dr. Crutchfield asked the patient whether the patient had any additional questions or concerns, and the patient indicated that the patient did not. With respect to the physician from whom the patient received subsequent care, Dr. Crutchfield does not find the complaint credible. Dr. Crutchfield considers the physician to be one of the most professional, experienced Mohs experts in the country. Dr. Crutchfield intends to continue to refer patients to that physician any time he determines Mohs surgery is appropriate.After review of the billed amounts, the charges were accurate for the services the patient received. No refund or adjustment to the bill is appropriate. We always appreciate a patient's time and attention in raising a matter with us and we use all feedback as an opportunity to review our operations and make improvements where necessary.
