concerns prior to receipt of this complaint, and we believe this matter has been resolved to his satisfaction. In short, the lab tests were appropriately bundled on the claim form [redacted]'s submitted to [redacted], and the claim was adjudicated (and pricing adjusted) in accordance with our provider contract. [redacted] subsequently requested an itemized statement of charges from our billing office. Given that it is an itemized statement, bundled procedures and labs are broken down into their component elements, but our patient representative explained to [redacted] that this is not how the claim was actually submitted to his insurance company. [redacted] can easily confirm that the lab tests were appropriately bundled upon submission to [redacted] by reviewing his Explanation of Benefits for this claim.
