Contacted business in November to receive payout of fsa account to make payment towards medical costs. Filled out pertinent informative on payout/reimbursement form as instructed. Business ignored information on said form and mailed out check to another address listed by my employer as a physical address where mail is not received. Contacted business about never receiving the check and instructed to fill out a missing check form. As a result this letter containing the check was stolen, fraudulently signed and cashed according to the results from the business. Asked for a replacement check to pay my medical costs and was denied, business said they were unsure if they could determine what was going to happen and made it sound like I’m pretty much out of luck.
After a lengthy process, *** Bank could not prove a fraudulent transaction occurred with ***’s cashed checkBased on the lack of proof that the transaction was not fraudulent as it related to ***, *** restored the funds, in the amount of $165.00, on 1/27/2020CompuSys/Erisa Group, Inc. re-issued the check to *** on 1/28/2020Based on the above, a satisfactory solution occurred in ***’s case. A benefit check from CompuSys was not processed by my bank and since has become stale dated. *** agreed in a phone call that Compusys would direct deposit the funds to my account by the end of business today. My bank *** states that at the end of business hours today, no deposit was initiated. I have been attempting to get this check reprinted since February.
A check, in the amount of $2550.00, was sent via UPS to Mr. Human Resources Department. It was sent to the following:

We use CompuSys ERISA as a third party administrator for payroll deduction for voluntary insurance. Lately we have had many many issues with them. The latest today they deducted several of our employees portion of insurance twice through payroll, one pretax and one post tax. This has happened before and probably will happen again. Please be aware of this company and stay in constant communication so your deductions are correct. Use another company if you have the option. Our experience is a very negative one. I give them two stars instead of one star because at least you can get them on the phone to talk to you.
