Review: In September 2014 I was charged $25 to have a medical form completed. Comprehensive behavioral health never completed the form. I contacted them numerous times about it. However, I had to have [redacted] internal medicine complete the form. So I went to comprehensive behavioral health and requested a refund since the never completed the form. I was told the office manager [redacted] would get back with me. As of yet I have heard nothing. I just wanted them to refund my $25 since they never completed the form.Desired Settlement: I wish for them to refund the $25 they charged me to complete a form that they never completed.

I am in receipt of the complaint filed by Ms. [redacted] on March 13, 2015 regarding a form she states wasnot filled out by our provider, Our office policy is that the patient must bring the form to theirappointment to have the form filled out. Ms. [redacted] was told that she needed to schedule anappointment to have this completed. She chose not to make an appointment for 2 month and only afterthat appointment d¡d she request a refund. The refund was sent to the patient and she has since cashedthe check. Please, let me know if I can be of any further assistance.Sincerely,
