My medical records are not necessary for this business to address the complaint The doctor and his relationship to this business were stated in the contract incorrectlyWhen it came time, before the week deadline for a refund, and I requested a transfer, I was not informed that this doctor was not, in fact, doing OB coordinate care with this businessThis business intended (I have this documented) to transfer my care late in my pregnancy without notifying me of their intent of this We signed this contact, in large part, because of the business' relationship with the aforementioned doctorIn fact, this was a deciding factor in our signing of the contract The things that can be addressed without medical records are : The incorrectly advertised relationship with the doctor in the contract Failure to inform us, as consumers of this business, that this relationship was not as promised in the contract when the transfer was requested Reason that the transfer was not carried out at the date requested before weeks If the business would like to provide proof that they did contact the doctor about coordinating my care with him (at the date that I requested a transfer), I will consent to releasing that, as that would be part of my medical records As I have previously stated, the Oklahoma Midwives Alliance Review Board is not an appropriate avenue for this complaint to be addressedThe board is made up primarily of the workers of this business and is therefore not an unbiased place to have this complaint reviewed and sorted out

My medical records are not necessary for this business to address the complaint.
The doctor and his relationship to this business were stated in the contract incorrectly. When it came time, before the 27 week deadline for a refund, and I requested a transfer, I was not informed that this doctor was not, in fact, doing OB coordinate care with this business. This business intended (I have this documented) to transfer my care late in my pregnancy without notifying me of their intent of this.
We signed this contact, in large part, because of the business' relationship with the aforementioned doctor. In fact, this was a deciding factor in our signing of the contract.
The things that can be addressed without medical records are :
1. The incorrectly advertised relationship with the doctor in the contract.
2. Failure to inform us, as consumers of this business, that this relationship was not as promised in the contract when the transfer was requested.
3 Reason that the transfer was not carried out at the date requested before 27 weeks.
If the business would like to provide proof that they did contact the doctor about coordinating my care with him (at the date that I requested a transfer), I will consent to releasing that, as that would be part of my medical records.
