I agreed to receive physical therapy services based on misinformation from an employee of CRHThey admit the mistake but will not write off charges
On 4/6/I visited the Gateway Road Physical Therapy location of Columbus Regional Health, having been referred there by my physicianBefore agreeing to receive services, I consulted with the office manager, ***, who assured me that my physical therapy visits would be covered by my insurance (***) 100% and that my only responsibility would be a $copay per visitI agreed to receive service based on that understanding; I would not have agreed to receive service if the cost were higherI received physical therapy services on that date and on 4/13, 4/20, 4/24, and 4/Each time I made my $copay with the understanding that it was my only responsibility, and I was never informed otherwise by the staff at the facility
