If you are a Medicare patient and have had issues with Colorado Sleep Institute, you can report them to Quality Improvement Organizations at 888-317-0891. The more they know, the more they can do about it. I contacted them with my concerns and they in turn contacted CSI, who in turn left me a voice mail. All of a sudden they're following up on my "February" order, which was actually placed in January. The part that has been "on the way" twice is now back-ordered (again, or still - who knows?), but they're happy to offer me a mask fitting appointment. Pfffffft! As if I would still be waiting for them, or trust them! What a lame attempt to save face. This agency called them on their BS, and that info is now on their record. If they start seeing patterns, they can take action against the clown show that is Colorado Sleep Institute.

Review of our medical record shows that Mr. [redacted] has never been seen by Dr. Evelyn Bolt. Additionally, the...
services cited were not processed through Colorado Sleep Institute but were transacted in Elan Sleep LLC. We ask that corrections be made to this effect. On July 9, 2015 prior to any of his subsequent appointments, Mr. [redacted] was provided with and signed a Privacy and Financial Policy which addresses Patient Payment and Insurance Payment Responsibility. Excerpt: Patient Payments: Co-payment, deductibles, services not covered by your insurance plan or outstanding balances are due at the time of your appointment. Payment may be made by: cash, check, Visa, MasterCard, Discover or American Express. Payment Plans may be approved on a case-by-case basis for financial hardship. Monthly recurring payments will require a credit card to be on file and may be subject to a monthly interest rate of 8%. Insurance Payments: In most cases, we participate and accept assignment of payment with specific insurance plans in the area on your behalf. When the correct insurance information is provided, we will submit your claims as a courtesy to you, our patient. Your insurance coverage is a contract between you and your insurance plan. You are responsible for unpaid balances left on your account regardless of the amount of your insurance coverage. Through the signed Financial Policy Mr. [redacted] was notified of his financial responsibility prior to ever being seen in our clinic and voluntarily decided to proceed with his subsequent office visits. After submission of his insurance claim the explanation of benefits report we received from his insurance company informed us that Mr. [redacted] had a financial responsibility of $50.00 for his office co-pay. Therefore, in accordance with our insurance contract a bill was submitted to Mr. [redacted] for his co-pay amount of $50.00. As a result, Mr. [redacted] is contractually obligated to pay this balance. We are contractually and legally obligated to bill patients for any outstanding balances they owe for services that we provide and that our allowed under their insurance policy. Insurance plans are a contract between the Payor and the plan holder and it is in all cases the patient's responsibility to know their specific insurance policy, including the deductible amount, co-pay and co-insurance amount under their insurance policy.
