seen in theindependently owned ClearChoice StLouis office and that is when they recommended she considerupgrading her upper bridge since she is experiencing a break down with those teethShe said she didn'tknow what to do because she can't afford any more work to be done on her teethI asked if she wouldbe in the Seattle area so we can evaluate and she said there isn't anything we can do to help becauseshe cannot afford it.ClearChoice Seattle (located in Bellevue, WA) is happy to see her for further evaluation and weencourage her to schedule an appointment so we can follow up directly with her stated concerns.Please contact me with further questions you may have

To Whom this may concern, Ms*** *** came to our office on August 21, for a free consultationAs part of the consult, she was presented with the price of treatment and quoted $6,She was then offered various funding options at which point she was given a laptop to apply
for fundingShe chose to use Care Credit, and was approved for the $6,loanMs***, decided at this moment that she would move forward with treatment (payment agreement attached)She was informed that the process to move forward with the dental implant surgery was an initial pre-surgery evaluation, then a payment in full to secure a surgery date at the end of her smile design appointmentMs***, was informed that the reason that we take payments in full prior to surgery was due to the extensive preparation between the lab, surgeon, and prosthodontist to prepare the case for surgeryDuring this pre-surgical appointment which was also performed on August 21,the Prosthodontist performed an extensive dental exam, a comprehensive medical review, took a series of x-rays, dental impressions, and prepared her surgical script for surgeryDuring her appointment she was presented with the final treatment plan that was again confirmed and signed by both her, and the doctor (treatment plan attached)Following this smile design appointment she proceeded to schedule her surgery date for October 4, She also ensured to pick up her surgical prescriptions at the pharmacy on this same day (in office record)On September 18, 2017, we reached out to her to reschedule her surgery date due to an unforeseen emergency the surgeon hadIt was at this point that she confirmed that October 26, 2017 would work for herThe next time we heard from Ms*** was on October 8th, at which point she advised us that she would not be moving forward with surgeryAlthough the initial pre-surgery was completedRight away, we processed the FULL refund of $and Care Credit confirmed on October 11, Amanda F* Operation Manager
