Review: On december 16, 2013, I received an eye exam for cataract surgery. On december 20 I received an envelope containing information to prepare for the surgery and I found this request to restrictive. I called [redacted] at ###-###-#### and asked her to send me a copy of the eye exam as I was going to try another eye doctor. [redacted] said she would mail a copy of the eye exam and as of today, January 2nd 2014 I have not received the copy. I paid for this information and I want it.
