My husband and I were not welcome according to the bartender at this location of the Mellow MushroomIn our past experience, other customers spoke profanities at us and my husband told the men that I was not a "***" and that we were just there getting pizzaDue to the fact that we were locals and non-regulars, the bartender attacked us with threats and preceded to gather men to make us exit the buildingSeveral months later, after "letting the incident go" we entered the establishment for a pizza and the same bartender said that we had to leave because we weren't from [redacted] The disrespect was overbearing and we will not give our business to any Mellow Mushroom from now on because of her righteous attitudeI'm from [redacted] and have been a bartender myself, however I have not criticized Arizona natives for their heritageThis is unacceptable behavior for any employee in a quality service industry

We need the Patients name and account number in order to look into this further

We mailed out 2 statements informing the consumer that we need Insurance Information and 2 statements letting her know that her Insurance denied the claim.

My husband and I were not welcome according to the bartender at this location of the Mellow Mushroom. In our past experience, other customers spoke profanities at us and my husband told the men that I was not a "[redacted]" and that we were just there getting pizza. Due to the fact that we were locals and non-regulars, the bartender attacked us with threats and preceded to gather men to make us exit the building. Several months later, after "letting the incident go" we entered the establishment for a to-go pizza and the same bartender said that we had to leave because we weren't from [redacted]. The disrespect was overbearing and we will not give our business to any Mellow Mushroom from now on because of her righteous attitude. I'm from [redacted] and have been a bartender myself, however I have not criticized Arizona natives for their heritage. This is unacceptable behavior for any employee in a quality service industry.
