Mr [redacted] has been a long-term patient of Chu Laser Eye Institute and has seen Dr Jessica [redacted] for his condition since Mr [redacted] eye condition requires very specialized contact lenses that are not covered by insuranceHis condition progresses and he has to update these special contacts every couple years Mr [redacted] payed for and picked up his new lenses on 3/6/during a very busy clinicIt was noted that he has called but the doctor was unable to return his call that dayThe next morning, 3/7/17, she called Mr [redacted] who stated to her that the lenses weren’t working and he paid for them He wanted to know what to do and didn’t want to continue to pay for new lenses Dr [redacted] reminded him that these special lenses can be fixed and tweaked by the manufacturer until the prescription feels good to him – at no additional chargeShe reminded Mr [redacted] that this is the process According to Dr [redacted] , Mr [redacted] felt relieved and was quite satisfied with the outcome of their conversation He has been in the office to see Dr [redacted] for subsequent office visits without issue and he continues to be a very valued patientPlease contact me directly with any further questions via email at [redacted] @chuvision.com or by phone at ###-###-####Sincerely, Carrie [redacted] Carrie [redacted] , COE

The next morning, 3/7/17, she called Mr. [redacted] who stated to her that the lenses weren’t working and he paid for them. He wanted to know what to do and didn’t want to continue to pay for new lenses.
