On 11/2/we recieved an EOB stating that patient was not eligible at time of service. At that time we reversed the original contractual adjustment. On 1/7/14, my biller named *** spoke with The patient's mother who stated that she had spoke with *** and they were going to send
the claim back thru for processing and we should hear something in to days. On 1/29/we recieved a new EOB stating that the patient wasn't an eligible dependent again. So the bill was then sent back to the the patient.Today, I had my biller call *** again to ask about the supposed incorrect name as we haven't heard anything about a name problem. She spoke with a *** who stated that the son and Father have the same name without the Jrthat was put on the patient's paperwork. We billed it according to the way the patient or parent filled out his information sheet. We also ask *** today to send out a corrected EOB according to the patent being eligible and we will write off the contractual adjustment.We always do our contractuals adjustments but I do need *** to send us a corrected EOB
