This is in response to your letter dated April 1, The above mentioned patient was seen on 2/19/The services were billed to *** *** which applied $to her deductibleShe was sent statements for this amount due on 3/9/16, 4/6/The patient was then seen on 2/26/and
3/25/for antepartum visits which would have been part of a Global Ob chargeThe patient then transferred her care to another physician on 4/26/These visits were then billed out separately to her insurance on 5/13/*** *** processed the claims and we received payment on 7/5/Additional statements were sent to the patient on 5/4/16, 6/2/16, 7/7/16, 8/10/16, 9/8/16, and 10/11/with no response from the patientI am attaching the EOB's from *** *** for your information.I hope this helps in resolving this matter.Please do not hesitate to contact me if you have any questions regarding the above matter,
