contacted by Mr. [redacted] and instructed not to accept funds from the nursing home. We contacted the nursing home and requested that all payments be stopped and we returned checks after that date that we received. February 8, 2016 Mr. [redacted] contacted our office and said he would purchase additional services and merchandise from our firm and he had the payments from the nursing home started again. November 1, 2017 Mr. [redacted] again contacted our office and asked if the payments could be stopped. We again asked the nursing home to stop payments.
Please note Guy & Allen does not have the authority to authorize payments for anyone front a nursing home. That authority lies with the next-of-kin, guardian or the power-of-attorney of the individual. It was explained to Mr. [redacted] that the funeral trust is irrevocable after it
has been in force for 30 days. The trust is transferable to another funeral home of his choice. Mr. [redacted] needs to contact the Indiana Funeral Director's Association, (800) [redacted] to make the transfer.
