The business is charging me for a teeth cleaning that I never had done.

I went to the dentist to have an oral exam on which they claim they also cleaned my teeth. They charged my insurance for a teeth cleaning on March 7th which was not performed. When I went to the dentist a second time on March 23 I had the teeth cleaning. when I went in the first time I had my oral exam and it was performed by the dental assistant. They are charging my insurance twice for a teeth cleaning. Each cleaning was charged at $96. They claimed they cleaned my teeth twice but the cleaning was only performed once. The receptionist Paula, would also give me false information. She claimed on the first day that a cancellation fee was $50 then when I called them a few weeks later to cancel an appointment with them for teeth fillings she claimed it would cost me $200 to cancel. She also gave me different quotes for the work provided every time I would go in to their offices. Their work was very unsatisfactory.
