This letter is to document follow up to consumer patient complaintThe consumer called the Revdex.com and was concerned that he had given his social security number for a back braceOn Thursday August 18th I contacted the person to discuss the complaintI explained the company's HIPPA policy and also offered to the patient to de-indentify his intake sheet by blacking out the social security numberI explained to the patient that it is necessary to obtain personal data to verify insuranceRecords were de-indentified and the patient was satisfied with my response to the complaintIn addition, the patient requested that my customer service rep write an apology letter, for not promptly respondingThis letter has been written and sentIn closing, I thanked the patient for the opportunity to improve our processThis complaint and follow up will be presented to the company Compliance CommitteeThank you
