On June 01, 2018. I visited the office of Caduces for my first OB visit. On this visit I was performed a pap smear. On June 22, 2018 I decided to go for my results as a walk-in and I was told by the office that there were no results and I also received a message from the medical staff Becca I that I was not given a pap on the day of my visit. I proceed to letting them now that this is not true and I need my results ASAP. I was informed by a staff member that she will go and ask the doctor who was there the same day. Cadeuces staff member returned to inform me that the Dr. Robert B, told her in his own words that" he absolutely did not perform any pap on me on the day of my visit. By this point I was waiting for 45 min and she then proceeded to say let me go with the Medical assistant who may have assisted him in the pap and staff member said (Becca I) said " she was never performed a pap on the day of her visit". At this point I became very alarmed and upset by then I waited for 1 hour and 15 min. Again, the caduceus staff said let me do some research and 14 min later she returns and gives me a pap result, and at this point I said I like proof that this is my pap results. Please provided me with a lb requisition form, and she said ok. one minute and she returned informing me that it was missing. Til this day June 29, 2018 the office will not produce my lab requisition, which is a state requirement for all specimens sent out to the lab. This process is mandatory to ensure specimens do not get mislabel or misplaced. I need my lab requisitions to in fact make sure these are my results.
This claim was thoroughly researched by our operational and executive team. All patient lab orders are tasked to the provider support team not the provider. The patient's chart did not reflect the order was requested by the physician ad this was discovered as an oversight by the staff. Since this complaint was filed the Caduceus for Women staff were able to call the lab, retrieve the report, and provide to the patient. The operational team also counseled all involved staff to assure this was an opportunity to train and improve the patient experience.
