The consumer's complaint did not contain enough information to identify the account that was the subject of her complaint. There is no account with a date of service for 12-04-14. The consumer has five dates of service placed for...

collection, the most recent being May 6, 2014. We had sent a request to all the credit reporting agencies directing that all of her accounts be deleted from her credit history on March 13, 2015 and have documentation of that
action. The consumer has claimed that she believes two of those services are related to workers comp issues.
The consumer previously complained to the Revdex.com on this matter and the Revdex.com assigned case #26059574 to that complaint. This previous case was directed to our client, Elkhart Emergency Physicians who replied on March 12, 2015 addressing the consumer's concerns and suggesting a course of action for resolution. To date I'm not aware of any follow up on the consumer's part. Our client's records indicate they have billed the employer for two dates of service, and called the employer to attempt to get further information regarding workers comp but did not receive the information they needed.
There is no current attempt to pursue payment from the consumer for the two accounts that she has indicated she believes are workers comp related, she does however have three other dates of service.
