I received your letter dated May 17, in regards to ID# 12160170. Mr*** is insured under the Employee Benefit Plan for Dicoese of La Crosse. Custom Benefit Administrators Co(CBA) is the third party administrator on the behalf of the self-funded Employee Benefit Plan for
Diocese of La Crosse. We received an accident claim on *** *** on April 14, 2017. The claim came in with a Person injured in unspecified motor-vehicle accident V89.2XXA. At the point of claim we sent an Accident Questionnaire letter to be completed and returned along with a Med Pay Questionnaire and request for Police Report. To date these were not returned by the member. This is standard procedure to ensure that the plan does not pay for claims when there is third party liability. Prior to receiving your letter, we were contacted by the broker’s office for this group and they requested since Mr*** was upset that we pay the one and only claim in our system as an exception payment.
