My complaint involves the billing statement I received from services at Benchmark Physical Therapy

On January 14, 2020 I mailed Benchmark Physical Therapy the following letter and asked for a written response. Unfortunately, I have not received any correspondence to date regarding my letter so I am left to file a complaint. I received another invoice on 2/29/20 and the amount due has not been adjusted.

To whom it may concern,
I received a statement dated 12/22/19 in the amount of $436.58 for 4 dates of service (11/14/19, 11/20/19, 11/22/19, 11/25/19) and contacted your billing provider, *** Rehabilitation for an explanation. I think I was misinformed when your staff, (Hannah) confused the $20 copay with the 20% co-insurance. I was under the impression that I only had to pay a $20 copay for each visit since you were in network with my insurance company. *** said my insurance pays 20% co-insurance.
I actually disregarded my Dr.'s referral to another well-known physical therapy group and chose your company since you were in-network. The $20 copay was quoted to me over the phone by Hannah and at each visit I paid $20. After I completed 4 services, and paid $80, I received the statement for $436.58. If only I could have received a bill after the first visit, I would have known to stop treatment in order to prevent the further 3 visits' bills. I further believe this to be a fraudulent, intentional business practice where patients are pushed to schedule many visits at the onset and billing is delayed.
Your bill for services averages about $150/ per visit for my out of pocket costs. If I knew this, I would have never made my first appointment, no matter how much pain I was in. I have been to many Dr.'s appointments (even specialists) that were less than $150/visit. My chiropractor visits were below $50 so I never would've thought physical therapy would be $150.
I have requested a breakdown of charges from *** Rehabilitation. *** could not adjust/reduce the charges but advised I contact you directly.
Will you please provide me some assistance by adjusting these charges to a more reasonable payment for me? Please note, I have not contacted the Revdex.com in hopes that we will be able to find a favorable resolution to address my concern. I ask that you please communicate in writing to me at the following address.

We regret that this consumer’s experience did not live up to our value of excellent communication.Our Customer Support and Clinical Management teams have reviewed the complaint and reviewed the consumer’s case. As a courtesy, they have agreed to waive the remainder of the consumer’s balance. If she has not already, the consumer will be receiving a confirmation letter of her $0 balance. It is our sincere hope that this resolution is satisfactory to the consumer.
