To Whom it May Concern,Mr*** was a member at our Gold's Gym Pocatello location for several yearsOn August 21,Mr*** came into Gold's Gym and put a temporary freeze on his account from that day toJanuary 18, (See attached form with his signature) On January 181h his account came
off of freeze,like it stats on the form, and his billing resumedWe also attached past freezes that Mr*** hadrequested on his account.There are no notes or signed documentation that he cancelled his account until July 25, On thatday, he came into the club, signed a cancellation form and filled out a cancel survey to waive the daynoticeAt that time, we processed his cancelation and cancelled his account anrl have had no furthercharges.Furthermore, MrPato stats that he wants refunded for months of chargesWe have no draft on hisaccount between September and December We are unsure where the months of chargesare coming from on his end, unless Mr*** pays for another account we have not been notified on.At this time, we want to ensure Mr*** is taken care of and has the best experience with us,but based on the information Mr*** has provided and signed, we are unsure where any errors wheremade on our end or what we can do to fix his issueWe do not show that we owe him months ofmembership dues as months ago, he clearly has check ins, in our clubWe also show his account isand was cancelled out with no further billing, the day Mr*** came in and signed the cancelationform.If there are any other requests or needs on this account that have not been brought to our attention ormaybe another account Mr*** has that may need cancelled, we would like to know so we can ensurehe gets taken care of.For any questions or concerns feel free to call or email me.Ari P***Director of OperationsGold's Gym of PocatelloFlandro Drive suite 200Pocatello, ID : 208-***@goldsgymidaho.com
