I have reviewed the response made by the business in reference to my concern, and find that this resolution is satisfactory to me.
The form I filled out included every piece of information he asked me for. If the company is going to ask for the information again on the phone, why bother putting it into the form?No need to answer that, as there will be no future contact. Thank you.

I called the gentleman who lodged the complaint, in response to a form he submitted, requesting we contact him to provide an insurance quote. He provided only a name, email address, phone number, and business name on a form, which is not enough information to generate...

an insurance quote. I asked if he could provide a few more pieces of information to complete the quote process, and he demanded that I just give him his "number." I explained that I needed just a bit more information about his business to return a quote, and again he demanded we just give him his number. At this point I said, "I'm sorry if there was some kind of misunderstanding; I won't be able to help you any further with this quote," and disconnected the call. Neither party used vulgar language at any time during this conversation. I apologize if there was a miscommunication as to the process of obtaining an insurance quote. After the exchange, I immediately flagged this person in our computer system as someone who is not to be contacted in the future, and sincerely hope he finds the insurance coverage he needs with a different company.
