Anthony H. Spann D.D.S. & Associates (Garfield), Inc.

This company mailed a letter to my late father in October of 2013. The letter was forwarded to me. My Dad has an $80.00 credit balance. I phoned the organization numerous times to have the funds due sent to myself. I was informed that since the patient had passed away they would have to forward the balance to the Ohio Division of Unclaimed Funds. Per a [redacted] at their facility, the credit balance was forwarded to the State of Ohio on 12/31/13. I phoned the State of Ohio twice (the last time was on April 3,2014) and the state has no record of the money being received.
