Anesthesiology Associates of Tallahassee failed to file a claim with my insurance for a procedure that would have been covered by insurance.
I received a collections bill from Law Offices of Mitchel D Bluhm for about $3904 for a medical procedure that I had August 9th, 2018, in which Anesthesiology Associates failed to file a claim with my insurance (Capital Health Plan). I never received any correspondence from Anesthesiology Associates after the procedure, which was 2 years ago. I always change my address with the post office and make sure my mail is forwarded so if they sent me something by mail, I would've gotten it. I also never received any calls. The collection agency has now reported the debt to my credit report, making a significant impact on my credit score.

Desired Outcome

I would like for Anesthesiology Associates to remove this debt or file a claim with Capital Health Plan like they should have done after the procedure in August of 2018.

Please be advised after thorough review of the account and the medical records providing to us by the facility so we may bill for the services rendered by our clinician, we have confirmed the patients health insurance information was never provided to us. The face sheet shows no insurance information therefore the account was billed as self pay. Prior to the account being sent to collections statements were mailed to the address listed on the account on 8/22/2018, 9/19/2018 & 10/17/2018. At which point since we were not contacted by the patient to provide any insurance information or make a payment arrangement regarding the balance on the account, the account went to collections. Since we were never contacted and provided the insurance information for us to submit our claim and at this point any claims submitted would be considered untimely filing, CAPIO now owns this account and any balance negotiations or further inquiries regarding this account will have to be made directly with CAPIO.

They never sent me any letters. My insurance was provided at the hospital. Every other provider involved in the procedure got paid with no issues. This provider's billing negligence should not be the patient's responsibility.

In reviewing the medical records, we have confirmed that they do not reflect any insurance information. If you would like to review the medical records we were provided for this account from the facility, please feel free to contact the facility directly and request they provide you with the medical records including the face sheet and they will gladly provide them to you.

Anesthesiology Associates of Tallahassee sent my bill to collections after receiving my payment in full. They will not return my phone calls.
over 2 years ago I received services from Anesthesiology Associates of Tallahassee. They sent me a bill for around $200. I set up a payment plan and my first payment was $50. I didn't receive another bill after that and started getting collections calls asking for twice the amount I owed. I decided to just pay Anesthesiology Associates of Tallahassee directly. The amount was withdrawn from my account, and I heard nothing for months after that. Then I started receiving collection calls from a different debt collector for same larger amount. I sent them proof of maybe using a screenshot of my credit card statement (since I never got a receipt) and then didn't hear anything back. 2 months later I started getting collection calls from a 3rd collections agency and they had no record of me paying or saying that I paid. So I tried calling Anesthesiology Associates of Tallahassee to get a receipt so I could get these guys to stop calling me. No one picked up so I left messages. It has been over a month and still no response. They have several google reviews with people saying they're going through the same thing thing. What's more, I don't see anything on my credit regarding this debt collection.

Desired Outcome

1. Give me a receipt of what I paid. This means you'll have to actually call me back. 2. Reach out to the debt collector and help correct his mess you've made. 3. Correct how you handle your billing.

Please be advised we were only able to locate 2 accounts pertaining to this patient. After a thorough review of both of the accounts, we were unable to locate any record of either of the accounts ever having been sent to collections. We have also confirmed that both of the accounts show they are at a $0 balance and paid in full. As per the patients request we will gladly have a current statement mailed to the address provided on this complaint for each of the accounts confirming the $0 balances on the accounts we located. If upon receipt the patient determines there are not the accounts she is referring to, please feel free contact our Customer Service Department anytime at X-XXX-XXX-XXXX and a Customer Service Representative will gladly assist you. Attached is a screenshot of the collections information on my credit report. If their records show I paid in full, then why was I sent to collections? They need to contact the collections agency and fix this. Sending my statements isn't enough.

Please be advised that we have again attempted to locate an account pertaining to this patient and are still only able 2 locate the 2 accounts we originally referred to in our initial response to this complaint. As previously stated both of the accounts we have for the patient were paid in full and are at a $0 balance. Neither of the accounts were ever sent to collections. We have reviewed the screenshot provided by the patient showing the collection agency entry on their credit report. Please note, that Asset Care, LLC is not one of the collection agencies we work with. As previously promised we have mailed the patient a copy of the updated billing statements pertaining to each of the accounts we located which reflect the account was never sent to collections and are at a $0 balance. Moving forward, we would suggest the patient contact the collection agency and request information regarding the original creditor including the original account number. Once they obtain that information, if they can provide us with the account number we will gladly search again if it pertains to us and attempt to resolve the issue for the patient. However, please note that our accounts billed under Anesthesiology Associates of Tallahassee either end in AI or GE. If the account number for the original creditor provided by the collection agency does not end in AI or GE it is not ours. Thank you in advance for your time and attention regarding this matter.

1. Just spoke to Asset Care LLC and they said they purchased this account on 4/8/2019.

2. Prior to Asset Care LLC contacting me about this account, I spoke to Frost-Arnett. Frost Arnett says they have no balance for me.

3. I need proof that these accounts were taken care of with the correct account number on the statement. If possible please email it to me. ***@gmail.com

Please be advised that we mailed the billing statements for the only 2 accounts we were able to locate for this patient on 8/10/2020. Regretfully, we are unable to email them however, the patient should receive them in just a few days. As previously advised these are the ONLY 2 accounts we were able to locate for this patient and though we utilize Frost-Arnett for collections when necessary we have no record of these accounts ever having been sent to collections. Furthermore we do not utilize Asset Care, LLC as one of the collection agencies we send our delinquent accounts to. Though we have mailed the billing statements for each of the 2 accounts we located which confirm the balance is $0 and the accounts were not sent to collections, we cannot provide proof of an account being removed from collections if we do not have record of an account being sent to collections. The billing statements will reflect the account numbers for each of the 2 accounts, upon receipt please review the statements and compare the account numbers on those statements with the original creditors account number for the account being handled by Asset Care, LLC for confirmation they do not match.

I received a collections bill from Capio Partners for about $1500, I didn't under why or what for. I looked deeper into it and it was for Anesthesiology Associates of Tallahassee. I am so confused as to why I got a collections bill when I never once received an actual bill saying that I owed money. I wasn't even told after surgery and that was in 2016, so about three years ago. I always change my address with the post office and make sure my mail is forwarded so if they sent me something by mail, I would've gotten it. They never once called me and trust me they have my number, its been the same number since I was 14 years old. I'm just so upset with this whole situation, because they handled it poorly, very poorly. Never was I contacted about owing them money and now I'm in debt $1500 for something I didn't even knew existed!!!This place is the worst and something needs to be done about it. This has happened to multiple people, its all over the reviews for this company.

Desired Outcome

Other (requires explanation) I would like the amount to be adjusted to a way lower amount since I was never notified that I actually owed money. I am not willing to pay $1500 when I didn't know I owed money. It would be different if I would have been told that I had an outstanding balance after my surgery was done, but I wasn't informed.

Please note that we are eager to assist the patient with their concerns however in order to do so, we first need to locate the account. My maiden name is *** That's the name that I'm under for the collections bill. The only account number I have provided to me is the one for my collections bill. I don't have any account numbers for anything else since I never received an actual medical bill from Anesthesiology Associates of Tallahassee before being sent to collections.
