I have reviewed the response made by the business in reference to complaint ID [redacted], and have only now been made aware of the Acord Lost Policy Release form. I have requested the company to email or send the form and I will sign, have my new insurance agent sign it and return it promptly. This resolution is satisfactory to me.

I am responding to your letter dated 5/20/2015 regarding the above stated Homeowners Policy, in which the Insured is disputing the outstanding balance on their policy. The policy was cancelled for Non Payment of premium on 12/20/2014. We were not notified that the Insured placed coverage elsewhere...
until 2/25/2015, in which we received a copy of the declaration page with the new Company. However, per our Company guidelines we do require an Acord Lost Policy Release form be completed and signed by the Insured and Agent on record. We then notified the agent of the declaration page received from [redacted] and notified the Agent of the need for the required Acord Lost Policy Release document in order to cancel the policy back to the desired date. On 3/25/2015 the Insured's daughter contacted our office regarding the collection notices the Insured was receiving. We then advised her again that we do require and need a completed Acord Lost Policy Release and referred her to her Agent so she can complete such document. On 3/26/2015 we received a memo from [redacted] for discontinuation of coverage. Again, we advised the Agent of the receipt of the memo for discontinuation and that in order for us to cancel the policy we need a completed and signed Acord Lost Policy Release Form. To date we have not received the required document-the Acord Lost Policy Release form, hence the reason the policy has not been cancelled back to the requested date of 11/11/2014. Once Allegany Co-op Insurance Company received this required form, completed and signed by both Agent on record and Insured we would be more than happy to fulfill the cancellation request.
